Monthly price of Sanitas Más Salud in Madrid by age (2026)
Price per person per month with a single insured person on the policy, from Sanitas's official April 2026 prices for Madrid (postcode 28220). All three plans have the same full cover: hospitalisation, surgery, tests, high-tech treatments and dental care with more than 45 services. The only thing that changes is what you pay each time you use the insurance. The price goes in five-year age bands, and everyone within a band pays the same. With two or more people on the same policy the price per person goes down, as we explain further down.

Official prices of Sanitas, S.A. de Seguros, for Madrid (postcode 28220), April 2026. They apply to new customers with a start date between 1 April and 31 December 2026. They are per insured person per month on a single-person policy. They are net premiums: each payment also carries the 0.15% surcharge of the Consorcio de Compensación de Seguros, Spain's public insurance compensation fund. They vary with age, postcode and the number of people on the policy, so your exact price comes from the calculator. Más Salud, Plus and Óptima can be taken out up to age 75 and have no maximum age for staying insured. Copays as set out in the current product sheet: Sanitas sets the amounts each year in the policy's particular conditions. Sanitas is a registered trademark of Sanitas, S.A. de Seguros.
What happens to the price at each age and in each family
Every figure in this article is an official Sanitas price for Madrid from April 2026, per insured person per month. We worked them out with postcode 28220, which is Majadahonda. If you want to compare plans at one given age, our guide to the cost of health insurance in 2026 covers it. Here we follow the same person over the years, and families by how many people are on the policy.
Until 44, age barely moves the price
It's a scene we see again and again at the office: someone in their early thirties torn between signing up now and waiting a few years. They are convinced the premium jumps with every birthday. In that age range the numbers say otherwise. Between the 20 to 24 band and the 40 to 44 band, Sanitas Más Salud with no copays goes from €106.93 to €121.13 a month: €14.20 more over twenty years of life. From 30 to 34 it costs €115.43 a month and from 35 to 39 it costs €116.82 a month: just €1.39 between one band and the next. On the plans with copays the curve is just as gentle. Más Salud Plus goes from €76.11 to €92.19 a month and Más Salud Óptima from €65.48 to €77.53 a month.
At this age, what really moves the bill is not your birthdays but how much you use the insurance. At 35, choosing Plus instead of the no-copay plan saves you €29.94 a month (€359.28 a year). That is almost exactly the copay cap in the Plus product sheet: €360 per insured person per year, with the first six services at €0. If you rarely see a doctor, that difference stays in your pocket. If you go a lot, the cap stops your copays from spiralling. Óptima charges a copay from the first service but lowers the premium by another €12.07 a month. At the office we run these numbers with your real use over the past year, not with an average.
From 45 to 64: the stretch where it rises most
The call usually comes at 55 or 60, with the renewal letter in hand and a premium well above the previous year's. The explanation usually has two parts: the annual price update (which Sanitas announces before renewal) and the move into the next age band when it is due. On Más Salud with no copays you pay €133.94 a month from 45 to 49, €143.97 a month from 50 to 54, €176.83 a month from 55 to 59 and €226.82 a month from 60 to 64. The step from 55 to 60 is the biggest on the whole scale (€49.99 a month). Between 40 and 60 the premium rises by €105.69 a month (from €121.13 to €226.82 a month). After that the increase slows down: €259.53 a month from 65 to 69 and €282.38 a month from 70 to 74.
Signing up earlier doesn't freeze the price. The premium is recalculated at each renewal with the age each insured person has reached, as the general conditions state. What joining in good time does give you is reaching 60 with your waiting periods already served (on Más Salud the one for hospitalisation is 10 months). You also fill in the health questionnaire while there is still little to declare. At this age the comparison between plans changes too. From 60 to 64, Plus costs €45.03 a month less than the no-copay plan (€540.36 a year, well above its annual copay cap). And it costs only €10.64 a month more than Óptima, with the first six services free of copays. From 70 to 74, Plus and Óptima are €6.74 a month apart. That's why we always put Plus on the table from 55 onwards.
From 60, Más Salud and Único +60 are not the same insurance
A couple aged 62 and 60 asking for a quote in Madrid gets two very different figures: €420.04 a month on Más Salud with no copays (€336.64 a month on Plus) and €99.10 a month on Único +60. The next question is always why the gap is so big. The answer is simple: they are two different policies and can't be compared as if they were the same product. Único +60 is available from age 60, with no upper age limit and no health questionnaire. It costs €49.55 a month from 60 to 64, €55.02 a month from 65 to 74 and €63.05 a month from 75 to 85. It includes GP and specialist consultations in every specialty, simple tests, ultrasound, CT and MRI scans, simple therapies, basic dental care, video consultations and home services. It carries a copay each time you use it.
What Único +60 does not cover is precisely what costs the most: hospitalisation, surgery and high-tech treatments such as chemotherapy, radiotherapy or haemodialysis. Its cover also applies only in Spain, and video consultations carry the same copay as an in-person visit. It suits people who rely on Spain's public health system for hospital stays and want specialists and tests without waiting lists. It also suits those already past the entry age for Más Salud. If what you want is private hospital care with admission and surgery, your insurance is Más Salud: it can be taken out up to age 75 and has no maximum age for staying insured. At the office we prepare both quotes and tell you clearly what each one leaves out.
As a couple or a family, more people on the policy lowers everyone's price
A 35-year-old couple often comes to the office with two individual quotes of €116.82 a month each. Put on the same policy, they pay €108.17 a month per person. On Más Salud, Sanitas's official prices have three columns based on the total number of people on the policy: one for a person alone, one for two or three and one for four or more. Everyone on the policy uses the same column, whatever their age. At 35 that means €116.82, €108.17 and €97.35 a month per person; at 50 it is €143.97, €133.31 and €119.98 a month. Four 35-year-olds pay €389.40 a month on a single policy, against €467.28 a month on four individual policies: a difference of €77.88 a month.
Two details are worth knowing. The third person doesn't change the column: with two or three people you pay the same per person. It's the fourth who lowers everyone's price, when the policy is taken out with all four as new customers. Children aged 3 to 15 share one price: €79.16 a month in the two-or-three column and €71.24 a month in the four-or-more column. The two-or-three column is also the one for a single parent with one child: a 45-year-old mother or father with a 12-year-old pays €203.18 a month in total. Babies aged 0 to 2 have their own, higher band: €139.70 a month in the two-or-three column and €125.73 a month in the four-or-more column. Children under 14 join with their father, mother or legal guardian also insured, unless agreed otherwise. In the calculator you can add each person with their age and watch the total change.
Más Salud Familias pays off from three people
Many families ask us for “the family insurance” by name, and it isn't always the one that works out best. With two people, Más Salud Familias costs a little more than Más Salud. A couple aged 40 and 38 pays €226.20 a month against €220.33. A 45-year-old mother or father with a 12-year-old pays €207.51 against €203.18. A couple aged 62 and 60 pays €426.68 against €420.04. With three the sums turn around, and this is where the gap shows most: a couple aged 40 and 38 with an 8-year-old pays €263.81 a month on Familias against €299.49 a month on Más Salud (€35.68 a month less). With four, the family aged 40, 38, 8 and 5 pays €313.72 a month on Familias against €340.77 a month on Más Salud: €27.05 a month less (€324.60 a year). With five (aged 45, 43, 15, 12 and 6) the family pays €391.82 a month against €426.28 a month: €34.46 a month less. The copay versions follow the same pattern: Familias Plus costs more than Más Salud Plus with two people and less with four (€235.76 against €249.28 a month).
So what decides it is not having children but how many of you are on the policy. Más Salud Familias has its own price table for one, two, three and four or more people. With three it is cheaper than Más Salud at any age: from 35 to 39 it costs €95.55 a month per person against €108.17, and a child aged 3 to 15 pays €69.27 against €79.16. Some of the cover changes too. Familias extends psychology to 20 sessions with no waiting period (Más Salud includes 15 a year), plus 40 more for eating disorders. It extends speech therapy to 20 sessions that also cover speech defects, and it adds family home assistance. Its dental cover includes more than 40 services, against more than 45 on Más Salud. We work out both with your ages and tell you which one suits you and why.
Avanza and Accede: an almost flat price, but no hospital admission
When a young family is looking for the lowest possible price, the answer lies in another range: the policies without hospitalisation. Avanza costs €39.35 a month from 20 to 39 and €41.83 a month from 40 to 59; children aged 3 to 15 pay €34.61 a month. Accede costs €29.88 and €32.25 a month in those same adult bands. Here age hardly counts and neither does the number of people, because the price is simply added up person by person. On Avanza (always without hospital admission) a couple aged 40 and 38 pays €81.18 a month, the family of four €150.40 a month and the family of five €187.49 a month.
The difference from Más Salud is not one of degree but of type of insurance. Avanza covers unlimited GP and specialist consultations, high-tech tests such as CT and MRI scans, more than 400 procedures without admission (groups 0, 1 and 2 on the scale of the OMC, Spain's medical council), basic dental care, emergencies and video consultations. It does not cover hospitalisation. Accede stops at consultations, simple tests and ultrasounds, with no surgery or admission. Accede is only for people new to Sanitas: it isn't open to current customers or to anyone who was a customer in the previous six months. Both carry a copay each time you use them. Both can be taken out up to age 59. They fit if Spain's public health system covers your hospital care and what you want is no wait to see a specialist. If you want admission, surgery with a hospital stay and childbirth in private care, your insurance is Más Salud, and that's what we'll tell you.
Paying annually: how much you really save
Most people pay their insurance monthly out of habit, without ever doing the sums. At 35 on Más Salud with no copays, twelve payments of €116.82 add up to €1,401.84 a year. In a single annual payment it is €1,345.77 (€56.07 less), which is like paying €112.15 a month. Quarterly payment has no advantage: €350.46 every three months, exactly three monthly payments. The saving is proportional to the premium. In euros it shows more on a family policy or from 55 onwards, when the premium is higher.
There are two caveats. First, the 0.15% Consorcio de Compensación de Seguros surcharge is added to any of these figures on the bill: €116.82 a month becomes about €117.00 a month and the annual payment about €1,347.79. Second, the annual-payment discount applies to Más Salud and other Sanitas policies but not to Único +60 (its premium is the same whether you pay monthly or yearly). There you choose how to pay for convenience. If paying the year in advance suits you, ask us for a quote with both options and decide with the figures in front of you.

Common questions
Direct answers, the same we give at the office.
How much does Sanitas cost per month in Madrid for a 40-year-old?
On Sanitas's official April 2026 prices for Madrid, a person aged 40 to 44 alone on the policy pays €121.13 a month on Más Salud with no copays, €92.19 a month on Más Salud Plus and €77.53 a month on Más Salud Óptima. All three include hospitalisation and the same cover; what changes is the copay. If you join with your partner, the price per person goes down: at 40 it is €112.16 a month on the no-copay plan. Without hospital admission, Avanza costs €41.83 a month from 40 to 59, but it's a different type of insurance: consultations, tests and surgery without admission. Your exact figure comes from the calculator, with your age and your postcode.
How often does my price go up because of age?
On Más Salud the price goes in five-year bands (20 to 24, 25 to 29, 30 to 34 and so on up to 70 to 74, then 75). Within a band, age doesn't change the premium. Until 44 the steps are small: between €1.39 and €4.32 a month on the no-copay plan. From 45 they speed up. The biggest steps come when you move into the 55 band (€32.86 a month more), the 60 band (€49.99 a month more) and the 65 band (€32.71 a month more). The premium is recalculated at each renewal with the age each insured person has reached and with that year's prices; the figures in this article are those of April 2026.
How much does a couple pay for Sanitas in Madrid?
A couple aged 40 and 38 pays €220.33 a month on Más Salud with no copays and €165.80 a month on Más Salud Plus. With two people on the policy, both move to the two-or-three price column: €112.16 and €108.17 a month each, instead of the €121.13 and €116.82 a month they would pay on separate policies (€17.62 a month less between them). On Más Salud Familias the same couple would pay a little more: €226.20 a month. At 62 and 60 it is €420.04 a month with no copays and €336.64 a month on Plus; if you're weighing up Único +60, we explain it in this same article. These are Sanitas's official April 2026 prices for Madrid; yours is in the calculator with your ages.
How much does it cost to insure a family with two children?
For a family aged 40, 38, 8 and 5 in Madrid: €340.77 a month on Más Salud with no copays, €313.72 a month on Más Salud Familias, €249.28 a month on Más Salud Plus and €235.76 a month on Familias Plus. With four people on the policy everyone moves to the cheapest column, and children aged 3 to 15 share one price: €71.24 a month each on Más Salud and €65.25 a month on Familias. If insurance without hospital admission is enough for you, Avanza comes to €150.40 a month for all four. One note: under the general conditions, children under 14 join the policy with their father, mother or legal guardian also insured, unless agreed otherwise.
I'm 62: should I choose Más Salud or Único +60?
It depends on whether you want a private hospital. From 60 to 64, Más Salud costs €226.82 a month with no copays, €181.79 a month on Plus and €171.15 a month on Óptima. It includes hospitalisation, surgery and treatments such as chemotherapy or radiotherapy. Único +60 costs €49.55 a month at that age and €55.02 a month from 65 to 74. It doesn't ask for a health questionnaire, but it is a policy for consultations and tests. It doesn't cover hospitalisation or surgery and applies only in Spain; of the high-tech tests it includes ultrasound, CT and MRI. If Spain's public health system covers your hospital stays and you want specialists without waiting lists, Único fits. If you want admission and an operating theatre in private care, choose Más Salud, which can be taken out up to age 75. We prepare both quotes for you.
Up to what age can I take out Sanitas insurance?
It depends on the product. Más Salud in its three plans, Más Salud Familias and Top Quantum can be taken out up to age 75. The reimbursement policies (Más 90.000 and Premium 500.000) can be taken out up to 64, and Avanza and Accede up to 59. Único +60 works the other way round: it is available from 60 and has no upper age limit. According to their product sheets, none of them has a maximum age for staying insured once you're in. On the reimbursement policies the price goes up year by year, not in bands. Más 90.000 with no copays reimburses 70% of costs up to €90,000 a year according to its product sheet; in Madrid it costs €127.88 a month at 35 and €247.07 a month at 64. If you're 59 and interested in Avanza or Accede, or 64 and you want reimbursement, don't leave it until after your birthday: at 60 and at 65 you can no longer join.
Is it worth paying for the insurance once a year?
On Más Salud, yes. For the family aged 40, 38, 8 and 5, the €340.77 a month adds up to €4,089.24 a year in monthly payments and €3,925.67 in a single annual payment: €163.57 less. For one person aged 35 the saving is €56.07 a year. Quarterly payment saves nothing: it is just three monthly payments together. On Único +60 the premium is the same however you pay. The 0.15% Consorcio de Compensación de Seguros surcharge is added on the bill to all these figures.
Is the price I see in the calculator the one I'll pay?
It's Sanitas's official price for your age, your postcode, the number of people on the policy and the payment method. The office adds nothing and charges nothing for its service. There are two caveats. The bill also carries the 0.15% Consorcio de Compensación de Seguros surcharge, so €116.82 a month becomes about €117.00 a month. And new policies on the health plans in the promotion get exclusive discounts until 1 February 2027; we confirm them when we prepare your application. If you already have an official quote from Sanitas or from another office for insurance with hospitalisation, bring it and we'll improve on it.
