Which is the best health insurance in Spain in 2026?
There is no single best one for everybody, but there is a best one for each person, and it comes down to four questions: whether you want hospitals owned by the insurer or a partner network is enough for you; whether you will want to choose a doctor outside the network, because then the product you need is the reimbursement one and not the basic plan; whether the dentist matters to you, because only Sanitas and Mapfre include it as standard, and to very different extents; and how much waiting period you can live with, especially if you are coming from another insurer and can ask to have it waived. With those four answers the table reads itself. At the office we go through that exercise with you with your current policy on the table.
Which insurer has its own hospitals?
Of the six, Sanitas is the only one with hospitals owned by the insurer: five in operation, three of them university hospitals, La Zarzuela, La Moraleja and Virgen del Mar, plus the fully digital Valdebebas in Madrid and CIMA in Barcelona. ASISA has the 18 hospitals of the HLA group, but they are shared: the websites of its own HLA hospitals state that they work with the main insurers, Sanitas included. Mapfre has 16 own medical centers, which are outpatient, and around 300 partner hospitals. Adeslas, DKV and Caser provide hospital care through a partner network.
Which insurance includes the dentist?
Sanitas Más Salud includes basic dental as standard and, if you want more, Dental Milenium or Premium as a supplement, with 220 own dental clinics and no health questionnaire. Mapfre Salud includes a shorter basic dental: a treatment proposal, one cleaning a year, X-rays and simple extractions. With Adeslas, ASISA and Caser, dental is a separate module with its own premium or a price per treatment, and with DKV the dental network works with a copay for each treatment, with 152 treatments at a published price. If the dentist matters to you, read the dentist row before the price.
Which one has the shortest waiting periods?
For hospitalization, the five competitors publish 8 months and Sanitas publishes 10 for surgical groups III to VIII, with 3 months for outpatient surgery and 8 for childbirth. We say it as it is because that is what the terms and conditions say. The practical difference is elsewhere: if you come from another insurer, with Sanitas you ask for all the waiting periods to be waived except childbirth, and Sanitas decides when it processes the enrollment. Caser also waives them with a year’s seniority at the previous company. In practice, anyone switching insurers with the waiting period already served starts without it; anyone taking out insurance for the first time does not.
Where does invoice reimbursement sit at each company?
At none of the six is it in the basic medical-network plan. With Sanitas it is two dedicated products, Premium 500.000 with 90% up to €500,000 a year and Más 90.000 with 70% up to €90,000, and Top Quantum includes it as standard in the family specialties. With Adeslas only Plena Extra, at 80%. With ASISA it is ASISA Reembolso, at 80% or 90% with caps from €100,000 to €240,000. With DKV it is Mundisalud, with Caser it is Salud Prestigio and with Mapfre it is the Seguro de Reembolso. If anyone sells you a basic plan saying it has reimbursement, ask for the terms and conditions.
Up to what age can I take out each insurance?
Sanitas accepts enrollments up to age 75, with no maximum age for staying insured according to its product documentation, and the Único +60 plan has no upper limit from age 60. DKV publishes 75; ASISA 70 in Completa and Reembolso; Caser 69 in Salud Integral and Salud Médica; Mapfre 65 in the plans with hospitalization, and its 75 applies only to Salud Esencial, which does not cover admissions. Adeslas refers in its information notes to the insurer’s admission limits, so its age limit is confirmed at enrollment.
Why don’t you put prices in the comparison?
Because a price can only be compared with another one built on the same basis: same province, same age and same copay version. Each insurer quotes on its own basis, and putting the Sanitas price next to an empty cell would do Sanitas a disservice and mislead the reader.
