Sanitas vs ASISA: which to choose in 2026?

Short answer: Sanitas stands out for its own university hospitals, dental included as standard and entry up to age 75; ASISA also has a hospital network, but a shared one: the 18 hospitals of the HLA group treat the rest of the insurers too, Sanitas included, and keeps reimbursement in a separate product, ASISA Reembolso. These are the facts we check before sitting down with a client: the real size of the medical network, own hospitals, dental, reimbursement, copays, waiting periods and enrollment age, using the official 2026 documents of both companies. We publish no prices from either, because they depend on province, age and plan. Every point is here, whether or not we come out ahead on it.

The ranges: what is being compared

Sanitas’s health range is organized in three tiers. The base plan is Más Salud, a medical-network policy with no copays; anyone who prefers a lower premium in exchange for paying something per visit has the Plus and Óptima versions, with their published copay schedule. For reimbursement, to choose a doctor outside the network, there are Premium 500.000 and Más 90.000, plus Top Quantum, which includes reimbursement in the family specialties. And at the entry level, Avanza, Accede and Único +60. When a client walks into the office, the first step is to place them in one of those tiers according to how they are going to use their insurance.

At ASISA, the YA, Esencial and Esencial+ plans are outpatient only and do not cover hospitalization; Completa, Completa+ and Completa++ are full plans that differ in their copay; and ASISA Reembolso has three levels. Reimbursement is not part of the Completa range: it is a separate product. The fair comparison is Más Salud against ASISA Completa, the full plan with no copays.

The medical network

Sanitas publishes more than 59,000 professionals and more than 4,100 own and partner centers, plus five own hospitals and 226 own dental clinics. It is by far the largest medical network of any we compare on this blog, and the only one in which part of the network belongs to the insurer itself.

ASISA publishes more than 50,000 professionals and more than 1,000 partner centers. It is a large network, the third-largest of those we compare, and the difference with Sanitas lies less in the number than in the part of the network the insurer owns.

Hospitals: own and group

Sanitas is the only one of the major insurers with hospitals of its own: five in operation. Four are in Madrid and are university hospitals: La Zarzuela in Aravaca, La Moraleja in Sanchinarro, Virgen del Mar and Blua Valdebebas, its first digitally native hospital, accredited as a university hospital in April 2026. The fifth is CIMA, in Barcelona. La Zarzuela and La Moraleja rank in the top 10 of Spanish private healthcare, and La Zarzuela is fifteen minutes from our office. Having your own hospital means insurer and hospital are the same house: no money up front, no reimbursement claims, and your medical record in a single place.

ASISA has the 18 hospitals and 37 medical centers of the HLA group, and here it pays to be precise: HLA belongs to the same group as ASISA, but its hospitals work with the market’s main insurers, Sanitas included, as well as occupational mutuals and road accident cover, as the hospitals’ own websites state. In other words, a Sanitas policyholder can also be treated at a partner HLA hospital; what ASISA lacks is a hospital exclusive to its own policyholders.

Reimbursement: choosing a doctor outside the network

If what you want is to choose a doctor outside the network, Sanitas covers it with two free-choice products: Premium 500.000 reimburses 90% of the bill up to €500,000 a year and Más 90.000 reimburses 70% up to €90,000. Both give access to a partner network in the United States, can be taken out up to age 64 and set no maximum age for staying insured. Top Quantum, in addition, includes reimbursement as standard in the family specialties. The base plan Más Salud, by contrast, works with Sanitas’s own and partner medical network, with no reimbursement.

ASISA Completa has no general reimbursement. To choose a doctor freely you have to take out ASISA Reembolso, which returns 80% or 90% of the bill with an annual cap of €100,000, €180,000 or €240,000 depending on the level. Against the €500,000 of Premium 500.000 the ceiling is lower, although for most families any of those limits is a long way off.

Copays

Más Salud has no copays: you pay nothing per visit, whether for a consultation, a test or a video consultation. Anyone who prefers a lower premium has the copay range, whose full schedule is published. The most detailed case is Avanza: the first six services of the year are free, the seventh to the fifteenth cost €5 and from the sixteenth onward €10; tests and therapeutic procedures are €12, emergency visits €8, and podiatry and dental €3, with a cap of €350 per insured person per year, so the maximum outlay is known in advance.

ASISA Completa has no copays. Completa+ charges €2.50 for general medicine, €6 for emergencies and admissions, €10 for an MRI, CT or PET scan and €9 per psychotherapy session, and publishes no annual cap; Completa++ charges €7, €14, €40 and €13 respectively, with a cap of €300 a year. The absence of a cap in Completa+ is the detail most worth bearing in mind if you use your insurance a lot.

Dental

Sanitas health insurance includes basic dental cover as standard, with check-ups and cleanings at its 226 own dental clinics, and for anyone who wants more, Dental Milenium or Dental Premium can be added as a supplement with no health questionnaire and no age limit for taking them out. It is the only insurer among those compared whose basic dental cover is provided in its own clinics.

At ASISA dental is not included: it is an optional cover with a separate premium and a per-treatment deductible paid at the clinic. In its 2026 schedule, a filling costs €50, a root canal €130, an implant €650 and orthodontics between €2,360 and €3,045. For anyone who only wants a check-up and a cleaning, the difference with Sanitas’s included basic dental cover is direct.

Waiting periods

With Sanitas there is no waiting period for consultations or emergencies; there are 3 months for outpatient surgery in groups 0 to II, 6 for complex tests and psychology, 8 for childbirth and 10 for hospitalization and surgery in groups III to VIII. What matters for anyone switching from another company is that all of them, except childbirth, can be waived at enrollment by providing proof of the previous policy; it is an underwriting decision Sanitas makes when processing the application, and at the office we always request it and confirm it in writing before you cancel the other policy.

ASISA applies 8 months for any hospitalization, childbirth included; 6 months for outpatient surgery, psychotherapy and MRI, CT or PET scans; and 8 months for transplants, of which it covers only bone marrow and cornea. There is no waiting period for life-threatening emergencies, provided you notify within seven days and go to the nearest center. For hospitalization, ASISA is two months shorter for a first policy; with a previous insurer, Sanitas’s waiver of waiting periods levels the field.

Enrollment age

Sanitas enrolls from age 0 to 75 and, according to its product documentation, sets no maximum age for staying insured. The Único +60 plan, designed for older clients, has no upper age limit for enrollment from age 60, something rare in the market.

ASISA enrolls up to age 70 in Completa, Completa+, Completa++ and Reembolso, and publishes no maximum age for staying insured. That is five years less than Sanitas, and a decisive detail for anyone approaching that age or insuring their parents.

Remote care and second opinion

With Sanitas, 24-hour video consultation is included in the policy through Blua, its digital services in the Mi Sanitas app, together with a second medical opinion. On Más Salud it carries no copay, because the product has no copays.

ASISA includes a second medical opinion and 24-hour telephone emergency assistance in the policy. Its video consultation platform, ASISA Live, appears on its commercial website but not in the terms and conditions, and that difference matters: what is in the contract is enforceable, what is on the website can change.

Where and with whom you sign up

With Sanitas you can sign up at a physical office with an exclusive agent, like ours in Majadahonda: the same person before and after enrollment, service in eight languages, no surcharge for the advice and a reply in under 24 hours. The price is the official Sanitas price, the same as through any other channel.

ASISA is sold through agencies, branch offices and intermediaries, with no exclusive office of the insurer.

Isn't ASISA better for having 18 own hospitals against the 5 Sanitas has?

It is not a comparison of numbers, because the 18 hospitals of the HLA group are not reserved for ASISA policyholders: on its own hospitals' websites, HLA publishes the list of companies it works with, and Adeslas, DKV, Caser, Generali and Sanitas are all on it, as well as occupational mutual insurers and road traffic accident cover. In other words, it is a hospital group that provides care to a large part of the market, you included if you hold Sanitas. The five Sanitas hospitals belong to the insurer itself and their activity is organised around its own policyholders; their websites publish no list of third-party companies, they only distinguish between policyholders and private patients. And on quality, three of the five are university hospitals, and La Zarzuela and La Moraleja are in the top 10 of Spanish private healthcare. What really decides it is not the hospital count but which centres and which specialists you have less than twenty minutes from your home: we go through that with you at the office, with your exact address, before recommending anything.

How much does health insurance with Sanitas cost compared with ASISA Completa?

We do not publish prices for either company, and that is deliberate: ASISA's premium depends on the province, the age and the plan, and ours comes from the Madrid rate table, one of the provinces with the highest premiums, so putting one figure next to the other would not be fair to either. What we do instead is the useful part: bring us your ASISA quote at our Majadahonda office and we will set it next to the Sanitas one for the same age, the same postcode and the cover you will actually use: own hospitals, dental included, medical network and co-payments. How do the medical network and hospitals of each insurer differ?

Sanitas has the larger medical network: more than 59,000 professionals and more than 4,100 own and partner centres in Spain, plus 5 own hospitals in operation: the university hospitals La Zarzuela (Aravaca), La Moraleja, Virgen del Mar and the fully digital Valdebebas in Madrid, and CIMA in Barcelona. Asisa works with a somewhat smaller network, around 50,000 professionals according to indicative market figures, but leans on its own HLA hospital group, with a strong presence in the Madrid region that many patients particularly value. No network figure is fixed for life: networks change as agreements are renewed, so what we do before recommending a policy is check that your doctor or your usual centre is still in the insurer's network at the moment you sign up, not just in the general table.

Which ASISA product includes reimbursement of bills, and what does Sanitas offer?

ASISA's Completa range does not include reimbursement of bills. To choose a doctor outside its network you need ASISA Reembolso, a different product that refunds 80% or 90% of the bill with an annual cap of €100,000, €180,000 or €240,000 depending on the level. It is the same with Sanitas: Más Salud works with the medical network, and free choice of doctor comes with Más 90.000 with Reimbursement, which covers 70% of the bill up to €90,000 a year. So the fair comparison is one company's reimbursement product against the other's, not one company against the other. Up to what age can I sign up with Sanitas or with Asisa?

Sanitas accepts new policies from 0 to 75, and from 60 onwards there is the Único +60 plan, which has no upper entry age at all. We have this figure very well verified, because it is one of the questions we hear most often from older relatives who come to the office. For Asisa we have not found a single, consistent published figure for the maximum entry age on its individual policy: the condition usually depends on the specific plan and on the commercial campaign running at the time. If you are over 65-70 and this is decisive for you, we recommend asking the insurer or agency for the exact condition directly before ruling out any option on grounds of age.

Is dental included in both of them?

In Sanitas yes, in ASISA Completa no, and that is one of the clearest differences in the table. Sanitas includes basic dental as standard in Más Salud, backs it with 226 owned clinics, and lets you upgrade to Dental Milenium or Dental Premium, with more than 40 treatments included at no extra cost and reduced fees on complex work like orthodontics or implants. In ASISA, dental is an optional guarantee with a separate premium and an excess for each treatment that you pay at the clinic: in its 2026 schedule, a filling costs €50, root canal treatment €130, an implant €650 and orthodontics between €2,360 and €3,045. When might Asisa suit me better than Sanitas?

The part that matters most is sequencing: the new policy must be in force before the old one lapses, with no gap at all, which is critical if you have a TIE or residency renewal coming up, because continuous cover is exactly what immigration checks.
