The exact price is calculated from three main factors: the age of each insured person (risk varies by age bracket), the postal code of residence (each province has a pricing zone based on local healthcare costs), and the number of people included on the same policy: the premium per person is lower with 2 or 3 insured and lower still with 4 or more.
The copay option (pay per use) has a lower fixed monthly premium: a 35-year-old adult in Madrid pays €116.82/month with no copays, €86.88 with low copays and €74.81 with high copays. It's ideal if you rarely visit the doctor during the year. If you want a fully predictable monthly cost, or you have young children, the no-copay option is usually the better choice.
Yes. The premium per person goes down when you insure several people on the same policy: lower with 2 or 3 insured and lower still with 4 or more. For example, with Más Salud with no copays in Madrid, a 35-year-old adult pays €116.82 a month alone, €108.17 each if there are 2 or 3 of you and €97.35 each if there are 4 or more. Paying for the whole year at once also costs less than paying monthly: that same adult pays €1,345.77 in one annual payment instead of €1,401.84 in twelve monthly payments of €116.82. The calculator applies all of this automatically.
Yes. The prices shown in our calculator are directly the Sanitas rates in effect for 2026. They are the insurer's direct prices, with no markup or commission added by our exclusive agency.
Private health insurance in Spain is exempt from VAT and from the Insurance Premium Tax (IPS). The only legal surcharge that applies is 0.15% for Spain's Insurance Compensation Consortium, a minimal amount of a few cents added to the base premium (the rates shown on this site correspond to Sanitas's net premium before this surcharge).
