Switching Health Insurance After 60: Up to What Age Sanitas Accepts You and What It Costs

Short answer: Sanitas Más Salud can be taken out up to age 75 and once you are in there is no maximum age for staying insured. Sanitas Único is taken out from age 60, with no maximum age. If you are coming from another insurer with at least a year on a full health insurance policy, you can ask for your waiting periods to be waived, and age is not among the requirements. In most provinces Más Salud costs €175.93 a month at age 60 with no copays, or €142.56 a month on the Óptima plan, on Sanitas's April 2026 prices. Here we explain what changes from 60, what the health questionnaire is like and how to make the switch before 30 November without being left without cover for a single day.

What changes in health insurance after 60

Every autumn people aged 60, 65 or 70 come to the office with their insurance renewal letter in hand. Almost all of them bring the same questions: whether they can still switch at their age, whether they will be dropped as they get older and whether they will have to wait again for an operation. The answers are in Sanitas's general conditions and its underwriting rules, and we give them to you with their source.

Up to what age you can take out each Sanitas policy

Sanitas's full health insurance, Más Salud, can be taken out up to age 75 on its three plans: no copays, Plus and Óptima. Top Quantum has the same limit. The reimbursement policies, Premium 500.000 and Más 90.000, which also let you see doctors outside the medical directory, can be taken out up to age 64. Accede and Avanza, which do not include hospitalisation, up to 59. If you are 62 and want free choice of doctor, you are still in time. If you are 68, your full insurance is Más Salud.

Sanitas Único works the other way round: it is taken out from age 60 and has neither a maximum age nor a health questionnaire. It includes GP and specialist consultations, tests such as ultrasound, CT and MRI scans, basic dental care, video consultations and home services, with a copay each time you use it. It does not include hospitalisation or surgery, so it suits people who want specialists without waiting lists and have hospital admission covered by Spain's public health system. In our guide to Sanitas prices in Madrid by age we compare the two policies at 62 with figures.

Once you are in, age does not take you off the policy

It is the question that weighs most from 65 onwards: “what if they drop me when I turn 80?”. At Sanitas there is no maximum age for staying insured, as its underwriting rules state. Age is not among the reasons for an insured person leaving the policy listed in the Más Salud general conditions: they are death, moving abroad or not living in Spain for at least six months a year, and serious misconduct towards staff.

The contract is annual and renews automatically every year. As with any insurance, both parties can oppose the renewal by giving written notice: you one month in advance and Sanitas two. And in its application forms (the general one used for Más Salud and those for Único and Accede) Sanitas puts in writing that it will not oppose the renewal for an insured person over 65 with 5 years or more at Sanitas and no unpaid premiums, nor for someone being treated for one of the serious illnesses it lists. That is why from 60 onwards it pays to join as soon as possible: every year you spend on the policy counts as seniority.

If you come from another insurer, you do not start again

Imagine someone aged 66 with ten years at another insurer and a hip replacement planned for the spring. On a standard Más Salud enrolment, hospitalisation and inpatient surgery have a 10-month waiting period, high-tech diagnostic tests 6 months and outpatient surgery 3 months. Consultations and emergencies can be used from day one, and in a life-threatening emergency no waiting period applies.

If you come from another insurer, Sanitas can waive those waiting periods. You need to have been on a private health insurance policy with another insurer operating in Spain for at least a year, with no gap between leaving one and joining the other. You prove it with the particular conditions or a certificate from your insurer and the last paid premium receipt, and age is not among the requirements. They are waived in the medical directory except for childbirth and bariatric surgery, not on reimbursement cover, and the waiver does not apply if you come from a civil service mutual fund (Muface, Mugeju or ISFAS) or from a reduced-cover policy. We request it at enrolment and you will know the result before you notify your insurer.

The health questionnaire: declare everything

Más Salud and the rest of the range ask each insured person to fill in a health questionnaire. At 60 almost everyone has something to tell: blood pressure, cholesterol, an old operation. Declare everything. If there is an illness, Sanitas assesses it and can accept the enrolment, accept it excluding anything related to that illness, apply an extra premium or not accept it. There is a list of conditions that do not need to be declared and we tell you what is on it when we fill in the application.

What is never wise is to keep something back. The general conditions exclude illnesses that predate enrolment and, if one is concealed, any cover related to it. It is the worst possible surprise just when you need the insurance most. The waiver of waiting periods for coming from another insurer does not erase pre-existing conditions either: they are declared all the same. Único is the exception because it is taken out without a health questionnaire.

What health insurance costs after 60

On Sanitas's April 2026 prices, in most provinces (Málaga, Granada or Salamanca, for example) Más Salud with no copays costs €175.93 a month at ages 60 to 64, €201.31 a month at 65 to 69 and €219.02 a month at 70 to 74. On the Plus plan it is €151.42, €174.85 and €193.86 a month. On Óptima it is €142.56, €164.81 and €188.24 a month. These are prices per person per month with a single insured person on the policy. Madrid, Barcelona, Valencia, Sevilla and other provinces have their own table: in Madrid Más Salud with no copays costs €226.82 a month at ages 60 to 64, and the full Madrid table is in our guide to prices by age.

The difference between plans is what you pay when you use the insurance. With no copays you pay nothing. Plus does not charge for the first six services of the year, then charges €4 to €10 per service with a cap of €360 per insured person per year. Óptima charges €4 to €15 from the first service. After 60, when people see the doctor more often, Plus is usually the middle ground that fits best, and we do the sums with your real use. Único costs €48.10 a month at ages 60 to 64 and €53.40 a month at 65 to 74 in most provinces, with a copay when you use it and no hospitalisation.

Bear two things in mind. The premium is calculated at each renewal with the age each insured person has reached and the rates in force, as the general conditions say, so it goes up when you move into the next age band. And these prices apply for the first year to people joining with an effective date up to 31 December 2026. If your new policy starts on 1 January, we confirm the premium when we prepare the application and always before you sign. With us you never pay more than the Sanitas rate, and on full health insurance we improve on your quote, guaranteed.

What gets used most after 60

Más Salud includes preventive medicine designed for this age: cardiology check-ups with a stress test when there is coronary risk, urology check-ups with the PSA test, digestive check-ups with the faecal occult blood test or a colonoscopy, and gynaecology check-ups. It has a medical second opinion for serious illnesses from specialists in any country in the world, special home care, and Blua's health programmes and video consultations.

Único adds home services with a set number of uses per year: medication delivery, physiotherapy, blood tests, podiatry, hairdressing, someone to go with you to medical appointments and overnight companionship if you are admitted to hospital. It also includes the Sanitas Mayores services, such as the personal care plan and support for dependency. We tell you which ones suit you before you choose.

How to make the switch before 30 November

If your current insurance renews on 1 January, you have until 30 November to notify your insurer in writing that you are not renewing: one month in advance, as set by article 22 of the Spanish Insurance Contract Act. The order of the steps is what protects you. First we compare your policy with the Sanitas one on the same basis. Then we request enrolment with an effective date of 1 January and the waiver of waiting periods. Only when Sanitas accepts the application do you send the non-renewal letter, which we prepare for you. That way you are not left without insurance for a single day.

Ages for taking out and staying on each policy according to the general conditions of each product and Sanitas's underwriting rules. Prices from the April 2026 rates of Sanitas, S.A. de Seguros, per insured person per month on a single-person policy, valid for the first year for new insured persons with an effective date between 1 April and 31 December 2026; the bill also carries the 0.15% surcharge of the Consorcio de Compensación de Seguros. Copays as set out in the current product sheet: Sanitas sets the amounts each year in the particular conditions. The waiver of waiting periods for coming from another insurer is a Sanitas underwriting policy: it is requested at enrolment and decided by Sanitas when processing it. Common questions

Direct answers, the same we give at the office.

Up to what age can I take out health insurance with Sanitas?

Más Salud, the full policy with hospitalisation, can be taken out up to age 75 on its no-copay, Plus and Óptima plans. Premium 500.000 and Más 90.000, the reimbursement policies, up to 64. Accede and Avanza up to 59. Único is taken out from age 60 with no maximum age, but it does not include hospitalisation or surgery.

Can I be dropped from my insurance because of my age?

At Sanitas there is no maximum age for staying insured, and age is not among the reasons for an insured person leaving the policy. The contract is annual and renews automatically every year. In its application forms (the general one used for Más Salud and those for Único and Accede) Sanitas puts in writing that it will not oppose the renewal for someone over 65 with 5 years or more at Sanitas and no unpaid premiums.

If I switch to Sanitas at 65, will I have waiting periods again?

If you have been on a private health insurance policy with another insurer in Spain for at least a year, with no gap, you can ask for your waiting periods to be waived, and age is not among the requirements. They are waived in the medical directory except for childbirth and bariatric surgery. We request it with your policy and your last premium receipt, and Sanitas decides when it processes the enrolment, before you notify your insurer.

I have an illness. Can I take out a policy?

Yes, by declaring it in the health questionnaire. Sanitas assesses it and can accept the enrolment, accept it excluding anything related to that illness, apply an extra premium or not accept it. The general conditions exclude illnesses that predate enrolment, and concealing them leaves everything related to them without cover. Único is taken out without a health questionnaire.

How much does Sanitas cost at 65?

In most provinces, on the April 2026 prices, Más Salud costs €201.31 a month with no copays, €174.85 a month on Plus and €164.81 a month on Óptima at ages 65 to 69, with a single insured person. Your price comes from the calculator with your postcode and we confirm it before you sign.

Which suits me better after 60, Más Salud or Único?

It depends on whether you want hospital care in the private sector. Más Salud is full insurance with hospital admission, surgery and high-tech treatments. Único covers doctors, specialists and tests with a copay, plus home services, but not hospitalisation or surgery. We prepare both quotes for you and tell you clearly what each one leaves out.
